Now that the one million woman study in UK has calculated its data, HRT has
been found wanting again. Pempro doubles the risk of breast cancer, while
estogen alone raises the risk about 30%. It is utterly amazing how
treatments become standard with self-selection as the basis for
decision-making. Once again too much medical care kills.

I thought he was correct and there are some REAL issues regarding hormone
replacement?

There are major changes underway with regards to prescribing hormone
replacement medications. I have discussed this before, but will summarize
by saying that although the Women's Health Initiative trial had a number of
design flaws (every study has some flaws), data from that trial suggest that
HRT leads to an increased risk of breast cancer, heart disease (mostly
within the first year of use), certain blood clots, and stroke. It appears
to decrease the incidence of colon cancer and osteoporosis and clearly
reduces hot flushes and vaginal dryness. For the purists out there, the
major issues with the WHI trial is that is really was not a primary
prevention trial. Many of the "healthy" women had conditions that may
predispose to cardiac disease, such as obesity and hypertension, and the
average age HRT was started was 63. Physicians don't prescribe HRT often to
women who are 63; we prescribe it to women just starting menopause, which on
average is age 51. Monkey research has shown that estrogen seems to prevent
atherosclerosis but will not remove it after it develops. So, perhaps HRT
would prevent heart disease if given at, say, age 50 but not at 63, or
perhaps the WHI was correct and HRT does not prevent heart disease at all.
Unfortunately, I doubt anyone will conduct the study that would answer that
question, which is a true primary prevention trial which starts HRT at age
50 and follows patients over time until the conclusion of the study. In
addition, it appears likely that HRT leads to an increase in breast cancer
that increases with the duration of use. Cardiac disease is about 10 times
more common than breast cancer, so "we" thought that the reduction in heart
disease would offset the small increase in breast cancer. Again, it would
take a different study to prove this, but the best we have is a huge,
randomized trial that suggests we should not use HRT for cardiac disease
prevention.

To put the WHI in perspective, it showed a 24% increase in cardiac disease.
That sounds like a lot, until you realize that out of 10,000 controls there
were something like 33 cases of heart disease per year. That's an increase
of 24%, which actually was about an additional 7/10,000 cases, for a total
of 39 out of 10,000. If you were one of those 7 women, that's a bad thing,
but 7 out of 10,000 is not an enormous number. To put this in perspective,
the absolute risk of a car accident in the US is about 800 per 10,000 people
per year. Hopefully absolute numbers like this will help put HRT into
perspective. I prescribe a number of medications with more risks than HRT.
However, many of the conditions HRT treats, such as hot flushes, may be
amenable to non-medical treatment. Obviously one would want to try proven
non-medical treatment as long as it appears safe.

I discuss HRT with my patients, just as I discuss other medications. I do
not prescribe HRT to prevent or treat heart disease, and I suggest that
patients who want to use HRT limit it to 1-3 years, if possible, because the
risk of breast cancer increases with time. Most patients with severe hot
flushes that are not amenable to non-medical therapy want HRT. Hopefully
they will be able to wean from it within a reasonable time period.
